What is Home Health Care?
Home Health was established by Medicare to help the elderly population remain independent in their homes or assisted living facilities as long as possible. Home Health Care provides services for which many of the elderly do not have access. It is for the patient who is home-bound, cannot drive far distances, or one or more. Many do not have an available caregiver to assist with personal care and nursing care.

When is Home Health Care
Requested?
A physician must order home health care for his patient when he or family members feel there is a medical necessity. When the order is received, an RN case manager will come to the home and perform an assessment. When the assessment is completed, a plan of care is developed and communicated with the physician. The physician and home health agency work closely together to assure the patient care is appropriate for the diagnose(s). The physician may order lab work, home health aides to assist with personal care, bathing, grooming, etc. Your physician could also feel it appropriate to order one or more therapies. The therapist will communicate to the RN Case Manager and the Physician always receives the
therapy Plan of Treatment provided by the therapist.
When is Select Home Health right for YOU?

Physical, Speech, or Occupational
"Home Care" is a simple phrase that encompasses a wide range of health & social services. These services are delivered at home to recovering, disabled, chronically, or terminally ill persons in need of medical, nursing, social, or therapeutic treatment and/or assistance with the essential activities of daily living.
Caregivers in your Plan of Care include:

Medical Socail Worker
Skilled Nurses
...will be included in your Plan of Treatment. The number of visits per week will depend on the assessment completed cy the RN Case Manager
Home Health Aides
During the initial assessment, the RN Case Manager will decide if you would benefit from Home Health Aide services and the number of weekly visits would be determined at that time. A Home Health Aide will be assigned to assist you with all aspects of personal care under the direct supervision of the RN Case Manager
Therapy Services
If the Physician feels his patient would benefit from therapy services, he could order Physical Therapy, Occupational Therapy, or Speech Therapy.
Medical Social Services
Provides patients and their families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses, such as Alzheimer's, Cancer, or AIDS. Services include advising family caregivers, providing patient education and counseling, and making necessary referrals for other social services.
